An intake health form is required to be completed before attending your initial appointment. If you provide an email address, this will be emailed to you once you have made your booking. If you do not have an email address the form will be sent to your mobile.

To access the clinic, go through the white gate and then enter the waiting room area on the left. Please note there is a step down into this room so take care.
